CHIP of Virginia is a statewide network of local programs committed to improving the health of Virginia's most vulnerable citizens. Teams of CHIP nurses and community health workers, or outreach workers, visit low-income families at home to ensure access to quality health care for their children and to provide health supervision and family support.
CHIP links young children from low-income families with health professionals who assure appropriate care in a consistent setting. CHIP assists physicians by prompting families to keep appointments, ensuring treatment recommendations are followed, and attending to the environmental and social factors that influence health and development.
In the CHIP model, outreach workers are members of the team that also includes nurses and family members. Together, the team helps to identify family needs and resources, and develops a plan to maximize the resources and meet the needs. Responsibility for implementation of the plan is shared among team members.
This oral presentation, to be delivered by Sandra Valdez, Lizette Ryan and a program administrator, will describe how the CHIP model works with families of limited English proficiency in Arlington, Virginia. Presenters will discuss CHIP's culturally appropriate, capacity-building approach to working with families, helping them set and reach goals to achieve self-sufficiency and to build parental confidence. We will also discuss strategies to strengthen families so they can support and encourage the physical and mental development of their children in a healthy, safe and stable environment.
