Temple Health Connection (THC) is a nurse managed, health care facility, with An eight year history of providing primary health care, including primary care, disease prevention, health promotion and family planning. THC sits within public housing and provides access to health care to those who previously had no primary care providers and used the emergency room for care. Advanced practice nurses, public health nurses, outreach staff and health professions students provide primary health care onsite and public health interventions out in the community. The history of THC is one of community partnership, where the community drives all education, service and research, directly associated with THC. This history of community partnership will be shared, along with the specifics of programs offered, numbers served, impact and outcomes, budget and funding sources. The long term sustainability for primary care and public health programming at THC will be projected.
