Richland Care: Health Care Delivery System for Low-income, Uninsured Persons

Richland Care is a health care delivery system that serves uninsured persons with incomes under 200% FPL and who reside in Richland County South Carolina. The program has been operational since November 2001 and has enrolled over 6,500 individuals. Richland Care participants are mostly African-American (72%), female (70%), and single (52%), and have incomes less than 100% FPL (55%). Participants have access to primary care, prescription drugs, subspecialty and hospital inpatient and outpatient services through a network of community hospitals, community health centers, private physicians and physician residency programs. Richland Care also offers a 24/7 RN Call Line and a Care Management services which targets high ED utilizers, and high risk participants who have hypertension and/or diabetes. Compared the their pre-enrollment utilization ED and hospital inpatient utilization rates, Richland Care participants have reduced their ED use by 74% and hospital admissions for ambulatory care sensitive conditions by 18%. This presentation will describe the program in more detail and will include financial planning information such as the value and types of reimbursed and subsidized health care services that have been provided to Richland Care participants and funding strategies.
