Interventions that Make a Difference: Innovative Programs to Improve Pregnancy Outcomes
This session presents a variety of programmatic solutions to reaching and retaining pregnant women in prenatal care as will as providing extra services for at risk populations. The sessions titled “ Maternity care coordination in a Medicaid managed care environment”, “Community health workers promoting healthy pregnancy outcomes through home visits” and “ Reducing rates of infant death and illness: a culturally sensitive model” highlight specific target measures to reducing infant mortality in vulnerable populations. The session “Screening for risk of diabetes among women of childbearing years” focuses on specific risk reduction strategies for one high risk condition of pregnancy. The session titled “Obstetrician-gynecologists and family physicians’ awareness and practices regarding folic acid” examines physician knowledge and resources related to folic acid recommendations to their patients. “Regional perinatal programs of California: improving pregnancy outcomes” describes their systems approach to coordinating services for high-risk pregnancies.
Learning Objectives: · Identify the key components of a maternity care coordination program and understand how to monitor the impact of these program components, and identify opportunities to improve maternal and child health in a Medicaid managed care environment · Describe how health education and support uniquely provided by Community Health Workers impact pregnancy outcomes of low-income and immigrant communities · Recognize the impact of community perinatal collaboration on perinatal outcomes · Identify factors that make the Black Infant Health (BIH) program a successful model of service delivery
