DESCRIPTION /GOALS OF THE PRESENTATION:
This presentation will describe and discuss the use of Community Care Coordination (community health work), the Settlement House approach and a formal helping system combined to create an effective strategy to address infant mortality in the African-American community. Community Health Work is locally recognized as Community Care Coordination. The presentation will be co-presented by a Community Care Coordinator (CHW).

At the conclusion of the session, the participants in this session will be able to:
1 Define the Settlement House approach and Community Care Coordination (community health work), while a local program
2 Identify effective components of an African American infant mortality reduction program
3 Recognize and describe a new and emerging Community Care Coordination Collaborative (community health worker network) and the benefits of these networks
4 Prioritize the training and networking of Community Health Workers
5 Articulate some of the unique health issues and psychosocial issues impacting African American women and their infants and how this approach addresses those issues
6 Analyze the advantages and challenges of this approach
7 Identify possible partners in their own communities to bring together and discuss this as a possible strategy to replicate
