Raising the standard of CBE skills among California primary care providers (PCP’s) is one strategy to decrease breast cancer mortality rates due to late-stage diagnosis. The Department of Health Services, Cancer Detection Section (CDS) implements the Clinical Breast Examination; Proficiency and Risk Management (CBE) training, a continuing medical education (CME) course for PCP’s as part of a breast cancer screening program. The course includes a 1:1 practicum session where PCP’s practice CBE core competency skills on a Patient Instructor (PI) and receive feedback from both the PI and a faculty clinician. Women from diverse backgrounds are selected and trained in the PI role. Training includes the ability to feel and assess skills and provide feedback using a “coaching method” that respects the learners’ current knowledge and practice. Based on findings from course evaluations that rank the PI feedback as one of the most valuable components of the course and effective in influencing provider behavior change, CDS has developed a PI Plan to ensure a high standard of preparation, self-efficacy and effectiveness among the 65 PI’s in the program. This presentation will focus on the methodology used to prepare and maintain highly skilled PI’s including effective recruitment, screening, training and evaluation strategies and program management. The plan creates an opportunity for community women to become involved in changing clinical practice and empowered to communicate valued feedback from the patients’ perspective to PCP’s. In addition, women become breast cancer screening advocates for themselves and other women in their community.
