In response to the need to collect consistent and timely data on clients involved with the Camden and Atlantic City Healthy Start projects, the Southern NJ Perinatal Cooperative developed a web based information system to enable community based case managers to monitor client activity and administrators to monitor progress towards program goals. Through the system case managers and administrators access client referrals, enter client intake forms, monitor client information, and track client progress through an established care plan. By placing this information in a centralized data base and case management system case managers are able to report on client activity and refer clients within the Healthy Start system. The web based system also provides the program with the ability to: •Gather, update, monitor and analyze client and system data to identify client issues and areas for improvement of coordinated program services. •Improve the consistency of services by evaluating information at the individual case level, agency level, and systems level. •Guide systems’ practices for continuous quality improvement. •Formulate clear policies and protocols within and between involved partners based on a data-driven process. •Improve the efficiency and effectiveness of all involved partners. The use of this system has vastly improved the amount and quality of data reported by case management staff.
