The Vision of Hope Pilot Project completed by the Illinois Eye Institute, Chicago, provided comprehensive eyecare to 1,700 patients in need in Chicago, through partnership associations with 17 community service agencies. The program goals were to identify and manage treatable eye disease, to promote early detection of serious disease, and to improve the quality of life through devices and patient education. The data collected through examination reveal a significant incidence of ocular and systemic disease. The information obtained through intake history surveys reveals great disparities in lifestyle and previous healthcare history noted by referral agency. The rate of systemic disease was reported as 45%. The average time lapse since last medical evaluation was 1.8 years, while the average time since last eye examination was 4.2 years. Of patients with significant systemic disease, notably diabetes and hypertension, 25% report having no current primary care provider and no plan for medical follow up. 56% of patients with diabetes have had no previous eyecare, while those who have received care report an average time of 2.5 years since last evaluation. In addition, the overall prevalence of diabetes varied by referral agency from a high of 100% to 6% with an overall rate of 14%. These statistics confirm the need for comprehensive care and intervention for those in need within the context of established community outreach. Future program success will be evident by preventing vision loss and minimizing complications from systemic disease within high-risk populations.
