Achieving measurable improvements in care and health outcomes through evidence-based population management

This workshop will focus on the key concepts and core tools necessary for developing, implementing, and measuring improvements achieved through evidence-based population management programs.

Kaiser Permanente's Care Management Institute will share the clinical methods, implementation learnings, and measurement expertise acquired over the past 6 years as CMI has developed and deployed 8 comprehensive population management programs. Kaiser Permanente (KP), which operates in 9 states and Washington, D.C., comprises 8.4 million members and over 11,000 physicians representing all specialties.

KP's Care Management Institute (CMI) has gained substantial experience through its comprehensive care management programs addressing the following KP populations: Asthma: 95,000 members; Coronary Artery Disease: 145,000 members; Depression: 480,000 members; Diabetes: 420,000 members; Heart Failure: 70,000 members; Elder Care: 800,000 members; Cancer (adjunctive therapies): establishing baseline; Chronic Pain: establishing baseline.

CMI has identified six core components of population management. The primary focus of the presentation will be on detailing these six areas and sharing specific examples of each: (1) Evidence-based Clinical Practice Guidelines, (2) Care Models & Tools, (3) Self-Management Tools and Approaches, (4) Outcomes Evaluation, (5) Technology Solutions, and (6) Implementation Support. CMI's sophisticated measurement capabilities have enabled demonstration of large measurable improvements, which will be shared with participants in this session.

CMI's population management programs, which have had broad impact within KP, can be customized easily for use in various health care settings. Likewise, the measurement activities can be adopted in other settings to define performance baselines and track progress over time.
