This session presents health-oriented strategies considered essential for inclusion in primary care today. Presenters will discuss projects and programs on weight and physical activity, oral health screening and education, and mental health assessment in conjunction with chronic illness care.
Learning Objectives: 1. Discuss critical gaps in typical primary care services, and 2. Describe projects and programs illustrating potential key primary care service enhancements.
