Since 1994, Alameda County has been undergoing a process to redefine and redirect the role of public health to look at a broad definition of health and well-being, and be more responsive to community priorities. One change that grew out of this process was the establishment of the Community Health Teams, a group of nurses and outreach workers stationed in select neighborhoods, who would mobilize communities to improve community well-being, in addition to their traditional roles as service providers.
As part of this on-going effort, the Community Assessment, Planning and Education Unit of the Public Health Department developed a set of Community Information Books for twelve neighborhoods in Alameda County. These books contain neighborhood-level information that describes the social-demographic characteristics and the health and quality-of-life issues that are affecting the community. Select indicators were chosen for analysis and presented in an easy-to-understand format. The books were designed for use by the Community Health Teams as part of an initial assessment process to understand the needs and assets of the neighborhood where they are based.
This presentation will describe the process of developing and using the Community Information Books as an example of using data in community mobilization strategies. Challenges and lessons learned will be discussed, including issues around small numbers analysis and the balance between community-generated data and public health department data. It will also describe specific plans for using the Information Books for community mobilization.
