The New Orleans Partnership for Care of the Uninsured (NOPCU), a public/private partnership of leaders in the health and social service sectors of New Orleans, was formed in order to address local concerns about disparities in health outcomes and lack of access to healthcare. Utilizing their collective resources, contributions from four Catholic healthcare organizations, as well as funds from HRSA's Community Access Program, the NOPCU resolved to develop a seamless health care delivery system for the medically underserved residents of Orleans Parish. A primary initiative of the NOPCU is the pilot testing of a high risk patient care management program which targets the "Status One" (1) patient population of New Orleans. Status One patients represent approximately 3-5% of the patient population who are at highest risk for immediate hospitalization, and who consume a disproportionately large portion of health system resources. Status One patients have a wide range of chief diagnoses that are routinely complicated by comorbidities and subclinical psychosocial complexities, all of which contribute to the risk of future high cost medical care. The pilot intervention is an integrated clinical and psychosocial program which employs care managers to coordinate clinical care; build self-reliance and self-efficacy; address mental and emotional needs; and strengthen social supports and community ties. The results of the evaluation of the NOPCU Care Management Program Pilot are at the heart of this presentation. The evaluation measures quality of life, continuity of care, and subsequent health care utilization (e.g., number of hospital admissions, days hospitalized, emergency room visits).
