Russia has one of the highest rates of abortion in the world--three out of five pregnancies end in abortion. Twice as many abortions occurring as births. There is widespread use of induced abortion not only for limiting the number of births, but for birth spacing as well. A study is underway in Perm, Russia to test models for providing services to postabortion clients so as to increase effective postabortion contraceptive use and to contribute to a reduction in the number of repeat abortions for women desiring to space or limit their family size. The study uses a quasi-experimental design using pre- and post intervention measurements. 1,518 abortion clients were interviewed before discharge and will be interviewed again at one year postdischarge on their fertility history (unplanned/unwanted pregnancy, abortion) and contraceptive behavior.

For the pre-intervention group (508 women) only 1% of the abortion clients left with a family planning method compared to 32% of the women interviewed post-intervention. The percentage of post-intervention women (1,010) leaving with a family planning method varied by site-—from less than 1% to 56%. Factors affecting access to and acceptance of family planning postabortion will be discussed and preliminary data from the one-year follow-up visits regarding fertility history and use of family planning reviewed.
