The recent investment of unprecedented federal funds for state and local bioterrorism preparedness promises to transform public health infrastructure. Yet, the traditional processes used to create such capabilities are unlikely to sustain effective community preparedness. CDC in partnership with NACCHO and other federal, state and local emergency response representatives, have developed a set of “Core Capacities” for local public health preparedness focusing on bioterrorism. We describe our three-year experience as local Center for Public Health Preparedness in developing a consensus set of roles, responsibilities, and operational action steps among response sector agencies serving a large (>3,000,000 population) metro-area. In order to leverage the separate activities and funding streams of various response sectors, we identify an integrative approach that 1) uses multidisciplinary, systems-based planning, 2) operationalizes “Core Capacities” into a framework for local implementation, and 3) builds training agendas and content on the basis of local plans and procedures. Our approach emphasizes systems-building, strategically identifying partners and desired outcomes and designating basic infrastructure building blocks that are created collaboratively. A scheme by which these activities may be prioritized is also featured. An integrative approach such as this helps select and sustain preparedness-building activities, and capitalizes on the entire local public health system to span across traditional jurisdictional and agency boundaries.
