Over 33 million Hispanics live in the United States with over 13% or 4.2 million living in Los Angeles County. This population faces significant health care issues with 30% of adults reporting poor to fair health. Many factors contribute to this adverse health status including no health insurance (46%), difficulty in receiving necessary care (47%), and lack of regular health care provider (36%). Language, health literacy, cultural and socioeconomic barriers further contributes to this public health problem. These structural and social barriers become increasingly significant in the presence of highly prevalent chronic conditions such as diabetes that require early detection, disease knowledge, and ongoing care to prevent development of complications. Pfizer Health Solutions and QueensCare, a community-based health care organization that provides services for indigent and underserved patients in Los Angeles, CA, have developed a community diabetes intervention and awareness program that aims to increase access to care, reduce health care barriers and improve the health status of local Hispanic residents. The program uses multifaceted channels to reach the local Hispanic community including an overarching mass media diabetes outreach and awareness campaign, use of culturally competent, low literacy health education materials based on the Transtheoretical Model and trained community health workers (CHW), collaborating with clinic physicians, to provide patient education, behavioral change counseling, monitoring and outreach to new onset diabetic patients and their families. Program implementation and formal evaluation methods including development strategy of patient education materials, CHW training curriculum, and mass media campaign will be presented.
