Increasing utilization and coordination of health care services through integrated data systems

In Montgomery County, Ohio the most significant barriers to accessing health care for the underserved continue to be lack of information about services and limited coordination of existing resources across service sectors. In order to better identify the uninsured and to facilitate connecting people with appropriate services, the HealthLink Miami Valley Collaborative has developed an integrated MIS system which combines health and social/human services encounter level data. Families without health care coverage who enter the public health system through either hospitals or social services agencies are identified and outreach workers assist them in accessing needed health care services. Aggregate data can be used for community wide health services planning, utilization review and disease management. Presenters will discuss significant elements in the development of the collaborative as well as the critical functions involved in integrating the health and human services data bases. These efforts which have resulted in increased utilization and more appropriate utilization of existing safety net services, can be replicated in communities across the country.
