In Kansas, the Healthy Start Home Visitor (HSHV) program is an integral part of the perinatal services continuum for county health departments and other local agencies. This project assessed the delivery process, intensity and duration, referral patterns, and diversity of services among county health departments. Client Visit Record (CVR) data for HSHV services from 105 county health departments (1997–2000) were analyzed, along with mailed-survey data (93% return rate). Using SAS and cluster analysis on quantitative data, CVR and survey results were analyzed to develop basic HSHV models of service delivery. To solidify understanding of these models, ten site visits were conducted. Qualitative analysis of site visit data further defined service diversity among county health departments. Results indicated six different program models used to meet individual catchment area needs. A detailed description of program model differences, similar program elements across all health departments, and how programs have adapted to their environment will be discussed. Project results will be used by the State to improve implementation and quality of perinatal services in Kansas. Findings also will lead to identification of quality measures and development of service standards in conjunction with the Kansas Association of Local Health Departments effort to formalize Maternal and Child Health certification standards.
