Problem: CDC data demonstrates a 60% reduced mortality and a 50% reduced hospitalization rate among males with hemophilia who obtain their care at a federally supported Hemophilia Treatment Center (HTC). However, California managed agencies often limit access to HTCs, and county Title V CSHCN case managers delay authorizations for HTC services. Purpose: To improve the health of Californians with hemophilia who are enrolled in managed care and State Title V Programs. Method: To expand seminars created in 1999 for 150 Title V case managers to include managed care agencies. The seminar presents clinical, program and outcome data, and consumer perspectives. Clinicians from California HTCs respond to composite case studies which highlight bleeding episodes and complications of a sibling pair from birth through adulthood. Findings: Fifty-five clinicians, from San Diego to Fresno, attended the September 2001 seminar. 55% from Title V and 45% from managed care. Over 90% completed the pre/post test evaluations. 100% rated the conference as good or excellent. Using a four point likert scale (4=high), participants rated the seminar from 3.13 to 3.49 in meeting its five objectives. 68% self rated their pre-test knowledge base in eleven clinical and program areas as some/moderate and 24% as good/advanced, vs. 11% and 81% respectively on the post tests. Conclusion: The seminar's content increased the hemophilia knowledge base of managed care and Title V case managers. It is a model State/Federal collaboration, addressing the health of children and adults with rare, chronic, catastrophic disorders, across insurance type.
