The S.A.G.E. Project (Summa Health System/Area Agency on Aging 10B, Inc.)

The S.A.G.E. Project is a collaborative partnership between a large urban area agency on aging and Summa Health System established in October 1995. This model integrates acute medical care services and the aging network of community-based long-term care services to provide a coordinated health care delivery system to reduce fragmentation of care and improve linkage to community resources. The historical “sector boundary” between acute and social care often results in less than optimal care. The S.A.G.E. Project attempts to “bridge the boundaries” between these two sectors. By integrating and collaborating, the agencies involved are able to assist frail, elderly individuals and their families achieve a higher quality of life by improving their functional status, by maintaining their independence in the community for as long as possible, and by avoiding or delaying institutionalization. The S.A.G.E. Project can be easily adapted to any organization and requires no additional funding or contractual agreements.
