The Maine Bureau of Health (MBOH) has been working with community coalitions for 16 years. Recently the Maine Bureau of Health received funding to conduct a comprehensive Cardiovascular Health Program to address five risk factors: tobacco use, physical inactivity, nutrition, high blood pressure and cholesterol. The Maine state legislature appropriated tobacco settlement funds to address risk factors in 31 communities, The Campaign for a Healthy Maine Community and School Program. Partnerships between the Bureau of Health, the Department of Education, the Maine Nutrition Network, the Maine Centers for Public Health and the Harvard Prevention Research Center were formed. A community assessment was developed by these groups and is being conducted in the 31 communities. This presentation will share the process of designing and implementing a community assessment through partnerships to cover this community and school program.

Learning Objectives: To increase knowledge of successes and challenges in conducting community assessments for physical activity, nutrition and tobacco through partnerships.
