Introduction: The HIV/AIDS Bureau of the Health Resources Services Administration (HRSA) administers the Ryan White CARE Act. The Title III Early Intervention Services Program in the HIV/AIDS Bureau provides funding for HIV Primary Care in community health centers, local health departments, hospital and university-based health centers, and AIDS service organizations. Twenty-six of 259 (10%) Title III Early Intervention Services Programs provide services in predominantly rural areas throughout the United States, District of Columbia and Puerto Rico.
Factors Influencing Provision of Rural HIV Care: Models of clinical service provision in Title III rural programs are diverse. Factors influencing the models of care include geography, regional epidemiology of HIV disease, population density, proximity to urban areas, and, specifically, proximity to university-based medical settings. Maintaining confidentiality in rural areas is a primary concern of patients and has been addressed in a variety of ways.
Models of Rural HIV Care: Comprehensive HIV Care is provided by local health departments, Community Health Centers, regional hospitals, satellite clinics of urban health centers and private physician offices. Most sites utilize a combination of direct on-site care and contractual relationships with private providers in rural areas. Rural sites typically link with universities for some specialty services and with community groups for advocacy-based services.
Summary: This presentation will describe the model of primary care in Title III rural programs, the mechanisms for monitoring quality of care, the impact of Medicaid managed care on Title III rural programs, and the provision of case management and social services in these settings.
