Objective: To identify the critical elements of various compliance enhancing strategies that can be applied across a multitude of behaviors and settings.
Data sources: Studies were identified by searching PUBMED(1990 to 2000) using the keywords: compliance, patient, communication, outcomes, health behavior theories, education and interventions. In addition to original articles, EBM reviews (via OVID) were gathered and a reference list of identified articles scanned.
Study selection: The studies were included if they included at least one intervention with an attempt to improve compliance with therapeutic recommendations; the study population had the characteristics similar to the general adult patient population; a control group was included; compliance with therapeutic recommendations or a health outcome was quantitatively measured; and sample size was 20 or greater.
Result and Conclusion: A total of 10,317 citations and abstracts, 603 reviews and 20 meta-analysis were screened from the database and bibliographic search yielding 89 articles, 21 reviews and 4 meta-analysis that met the selection criteria. The setting varied from doctors' clinics, community hospitals to advanced tertiary care centers. The studies cut across many diseases and behavioral recommendations.However, there was no single best strategy and in general, the combination of these strategies have been more effective than single- focus interventions. Based on the articles, seven critical elements of these programs have been identified and it is recommended that strategies should be made more comprehensive and broad based by incorporating all or most of the critical elements in order to be more effective and powerful.
