Nursing Challenges in a Program for At-Risk Elderly

The rapid growth of the elderly population has caught communities in the United States unprepared resulting in too few community-based health services to enable seniors to remain at home. Cost-effective, innovative, community-based services are needed to address this dilemma. Sentara Senior Community Care (SSCC) is a model program designed as an alternative to nursing home care. The program is modeled after a nationally replicated project called PACE, Program of All-Inclusive Care for the Elderly. Medicare and Medicaid grant special waivers for services, making SSCC a cost-effective alternative to nursing home care.

The program provides at-risk elderly with comprehensive care to minimize health problems and maximize independence. The program is a prepaid plan that provides total care for 110 participants, including comprehensive medical and rehabilitative services, in-home care, and transportation. Other services include adult day health care, prescription medications, primary and specialty medical care, outpatient medical services, hospital patient care, and nursing facility care. Most care is delivered in the Day Health Center.

The multidimensional aspect inherent in the advanced nurse practitioner's role makes this the ideal health care provider in a comprehensive care program. The advanced practice nurse plays a major role in effective pain management which is critical in maintaining strength and mobility, enhancing self-image, improving mood and increasing quality of life. Nutrition and weight management, also impacting on mobility and self-image are important nursing management components. Economic issues are considered daily as the nurse makes cost-effective decisions regarding interventions.
