The Los Angeles County Public Health Nurse Home Visitation Project (PHN HVP) uses the Dr. David Olds' model of Nurse Home Visitation, and provides services to first-time pregnant young mothers who are living in poverty. Several different funding sources are used for the PHN HVP, and each funding source has restrictions on the population that can be served. Linking the individuals served to the correct funding stream for over 700 clients has proven to be quite a challenge. The PHN HVP developed a computerized tool for staff to use in tracking client services that also helps to maintain the data necessary for Project financial accountability and evaluation.
The PHN HVP receives funding from the local Department of Public Social Services (DPSS) to serve those clients who are enrolled in their welfare-to-work programs. This funding stream mandates only DPSS-enrolled clients may receive services using this funding stream. The Project is also funded using net County funds matched through our State Department of Health Services, Maternal and Child Health branch, and also from funds received from the Proposition 10 Commission, that was established to disperse funds collected from a locally-applied $.25 cents a pack tax applied to cigarettes. Both funding streams utilize matching MCH dollars, and require close tracking of the populations served.
The application of this computerized method to track client services can assist similar projects track categorical funding streams while providing generalized, comprehensive services to varying client populations.
