This presentation will explore the evolution of a nurse-managed clinic in a rural community. The cultural determinants of care which affect the process of implementing a community development model will be described. The clinic was established in 1996 to provide disease prevention and health promotion services to uninsured and under-insured residents in a Medically Underserved Area (MUA) and a Health Professional Shortage Area (HPSA). Major accomplishments will be analyzed, including: resolving community resistance to implementing a sliding fee scale for payment; involving area hospitals in provision of services; achieving an effective provider mix, including a family nurse practitioner, community health nurses and community health workers; and negotiating clinic sites and services with community members and the Advisory Board. Ongoing research related to client satisfaction as an outcome indicator for nurse managed services will be discussed. Anecdotal accounts of the dynamics involved in building community partnerships and the strategies which have been successful in addressing the goal of eliminating health disparities in the community over a five year period will be incorporated.
