Temple Health Connection (THC), of the Department of Nursing, College of Allied Health Professions, Temple University, is a neighborhood-based primary health care clinic providing primary health care, family planning, disease prevention and health promotion for an indigent, underserved population in North Philadelphia, in partnership with the community. The community's need directs service, education and research at THC, with approval of our Community Advisory Board. THC uses advanced practice nurses and outreach workers to deliver care in collaboration with a neighborhood family medicine physician. In addition, undergraduate and graduate students from Temple University, as well as other educational institutions, participate through their formal education programs, as volunteers or as student workers. THC has recently expanded its services to provide summer day camps and an after school Homework+ program that includes health education and introductions to health careers. The demographics for the population served in the year 2000, as well as the number and type of primary care clients and public health encounters, program outcomes and funding sources will be shared.
Learning Objectives: to understand how a community's needs direct the service, education and research in the partnership described. to evaluate the outcomes achieved in the partership described.
