Objective: To present a systematic review and evidence-based recommendations on diabetes self-management training interventions in community settings. Methods: The diabetes chapter of the Guide to Community Preventive Services is being developed under the auspices of the U.S. Public Health Service to serve as a resource compendium of evidence-based recommendations for preventive health interventions for communities. Self-management training interventions in the home, schools, work sites, community gathering places, and recreational camps were examined. A conceptual model was developed of the relationships between the intervention, modifying factors, process and intermediate outcomes and health outcomes. A systematic review and quantitative synthesis was performed, and recommendations formulated based on the suitability of study designs, quality of the data, number of studies, and consistency and magnitude of the effect sizes. Results: There is insufficient evidence to recommend interventions in schools, camps and the work site due to a lack of quality data. Recommendations for the home and community gathering places are pending, and a meta-analysis of these results will be presented. Conclusions: Gaps in the volume and quality of exising scientific literature were identified, and point to the need for further research on community interventions in diabetes self-management training.
