The purpose of this session is to describe several exciting community-based and network-building projects to promote cardiovascular health in high-risk communities. The National Heart, Lung, and Blood Institute (NHLBI), NIH, adopted the Healthy People 2010 objectives as the foundation for program planning and development efforts. The NHLBI has also adopted the overarching goal of eliminating health disparities and focused on cardiovascular disease (CVD). CVD disproportionately affects several subgroups, particularly low SES and racial/ethnic populations. The NHLBI has been identifying high-risk CVD communities using geographic mortality mapping and is funding "enhanced dissemination and utilization centers" in these communities to eliminate CV health disparities. Several other projects focus on national efforts as well. An important aim of these projects is to build and strengthen communities' capacity to implement performance-based, results-oriented projects. These efforts are creating a shared vision among community partners about focusing on promoting heart-health. The NHLBI is providing resources to communities' to increase their chances of success. Moreover, efforts are also being expanded to enhance the dissemination of research findings to local communities. Based on NCQA/HEDIS data, if research results were applied more widely, many more lives could be saved. Thus, professional education training and distance learning opportunities using Web-based technology are also important project components. By engaging "networks" of communities, the NHLBI hopes to make "Cardiovascular Health for All" a reality by 2010 | ||||
