| Background The Racial and Ethnic Approaches to Community Health (REACH 2010) Demonstration Project is part of the DHHS response to the President’s Race Initiative and Goal for 2010 to eliminate disparities in health status experienced by racial and ethnic minority populations in six priority areas: Infant Mortality, Deficits in Breast and Cervical Cancer Screening and Management, Cardiovascular Diseases, Diabetes, HIV Infections/AIDS, and Child and/or Adult Immunizations. These are two-phase projects to foster community mobilization and the organization of resources to develop effective and sustainable programs. Phase I is a 12-month planning phase and Phase II - a four year implementation/evaluation period. The communities include: African American, American Indian, Alaska Native, Hispanic American, Asian American, and Pacific Islander. Strategy Success depends on a multi-pronged strategy at the local, state and national levels to develop, implement, and evaluate these projects. Public and private agencies, community coalitions, many State health departments and several DHHS agencies are partnering to reduce and ultimately to eliminate health disparities. Results To date, CDC has funded 32 community coalitions to work on Phase I community action plans in order to compete for Phase II implementation cooperative agreements. The California Endowment has provided funding to support an additional 3 coalitions in California. This panel presentation will highlight partnering at the Department of Health and Human Services, private foundations, universities, and at the local level for health improvements, identify research and actions needed to implement six important health indicators, and describe how community coalitions have been mobilized for action on health objectives | ||||
