Today, 70% of medical care costs are made on behalf of people with chronic conditions. Services for these people are financed by multiple public and private sources, each having different requirements for the administration, financing, and delivery of care. While care delivery is local, it is regulated by state and federal government, and is financed by both state (Medicaid) and federal (Medicare and Medicaid) dollars. As a result care for people with chronic conditions tends to be fragmented, institutionally-based, and reactive. Beginning in 1990s, with support from the Robert Wood Johnson Foundation (RWJ), a number of provider and state-initiated demonstrations aimed at developing models for integrating the delivery and financing of care have been worked on. In this session we provide an overview of the RWJ Medicare/Medicaid Integration Program, and present findings from three demonstrations. Minnesota Senior Health Options (MSHO) is the oldest of these projects having started enrollment in 1997. In the session, the early outcomes from MSHO will be presented and discussed. The Continuing Care Network Demonstration (CCN) sponsored by New York State has received its Medicare waivers in September of 1999, and is expected to become operational in 2001. The session will examine the CCN project with a particular emphasis on its innovative payment model and on the challenges of program implementation. Massachusetts Senior Care Organization (MSCO) is the newest of the three demonstration. MSCO has recently sponsored a series of outreach focus groups with consumers and providers of care, the results of which will be presented and discussed in this session | ||||
