Healthy People 2010 national goals are to have 95% of children up to six years included in a population-based immunization registry as a means of assuring high immunization coverage levels, especially for children at risk due to socioeconomic status. California DHS, in support of this goal and has funded developing or operational registries in 17 communities. The trial and error registry development environment that has prevailed nationally, along with shrinking sources of funding. jeopardize full registry deployment. To sustain this tool for reducing disparities to access to preventive health services, California conducted a rigorous evaluation of local registries and identified a number of strategies to deploy registries more widely and cost effectively. They are now targeting and leveraging their limited funding for: continued support only for immunization software for selected county products; development and enhancement of Contra Costa software to replace failed systems and for counties which have no registries; contracting with and providing start up funding to a 3rd party vendor to assist in the deployment of the Contra Costa County registry software in other counties; Web enablement of registries as the most promising and cost effective strategy for private provider participation; piloting of HL7 standard for bi-directional record interchange between providers and local registries and intrastate via a state hub; and strong promotion of regional deployment both to better coincide with health service delivery and to achieve cost savings by consolidating operations. We will present the processes and strategies and their impacts on expanding registries.
