This presentation describes the development of the Southeast Community Action Planning Project. This demonstration project was funded by the National Center for Chronic Disease Prevention and Health Promotion (CDC) under the Racial-Ethnic Approach to Community Health (REACH) 2010 federal initiative. A partnership was formed by the Southside Health Consortium, the Midwest Latino Health Research, Training, and Policy Center (University of Illinois at Chicago), the Illinois Diabetes Control Program (IL. Dept. of Human Services), and others to address disparities in diabetes prevention and control, adult immunization, and other chronic disease risk factors. The mission, goals, values, and participatory processes are examined and evaluated. Development of infrastructure and membership expansion is discussed.
