Establishing partnerships between the faith community and the health community can be an effective mechanism for addressing complex psychosocial health problems. However, there is a need to better understand the various ways in which partnerships can occur between these two communities. An understanding of these partnerships can contribute to better planning and evaluation of the programs implemented by faith and health partnerships. The Faith Health Partnership model has been developed to provide such a framework. At one end of the framework spectrum are health-led partnerships, in the middle are shared leadership partnerships, and at the other end of the framework spectrum are faith-led partnerships. A full description of the model and practical examples illustrating the various partnerships will be discussed. Criteria for identifying what types of partnerships may exist in a given community will be covered. Additionally, the Faith Health Partnership model will be discussed for its implications in improving practice.
