In recent years, the nation’s health care system has been guided by market forces that control the distribution of health and health care resources. Many communities are seeking to shift control locally to ensure the use of local resources in a more effective and efficient manner. The emergence of public-private partnerships characterized by cross-sectoral collaborative efforts among public health, social services, hospitals and health systems, and other organizations is proving to be an effective solution. Such partnerships promote shared accountability and greater focus on the health of the community through combined use of resources.
This session will highlight the efforts of two community partnerships, Vision for Health Consortium in Baltimore, MD and Lincoln County Public Health Coalition in Davenport, Washington. These partnerships are implementing the Community Care Network (CCN) vision based on the four dimensions of community health focus, community accountability, management within limited resources, and seamless continuum of care. The CCN Demonstration Program encourages health care providers to work with other local organizations, institutions, and community residents to improve the health care delivery system, enhance local support systems, and address the underlying causes of health problems in the communities. We will describe the strategies to address institutional barriers related to cross-organizational planning. The collaborative relationship between the local health department and health systems has lead to reduced duplication of services and enhanced quality of services and programs focused on locally defined needs. The partnerships will share lessons learned for sustaining a collaborative infrastructure that strengthens interorganizational and intra-organizational relationships.
